  1. BMI = body mass index, SBP = systolic blood pressure, DBP = diastolic blood pressure, LV = left ventricular
  2. Smoker: Self-reported smoker within the past year.
  3. Diabetic: current treatment with insulin or oral agents OR a fasting glucose ≥126 mg/dL.
  4. LV Hypertrophy: sex-specific thresholds; LVMI ≥51 g/m2.7 for males, LVMI ≥49 g/m2.7 for females
  5. Hypertensive: previous clinical diagnosis by physician with current anti-hypertensive treatment, OR an average SBP ≥140 mmHG or DBP ≥90 mmHG on the second and third clinic visits.
